An administrative error resulted in the July Monthly Operating Report being submitted to CDPHE two days past the deadline.
A robust root-cause analysis was performed, resulting in identification and implementation of multiple process improvements, including addition of verification of submission by a second staff member, as well as measures to manage the impact of work absences of report reviewers on the review, approval, and submission process.
